Does the evaluator not have the paperwork? I'm in Florida, and the evaluator I used had all of the paperwork ready before I got to her house. I would call your evaluator to see if she has the paperwork, or can tell you where to get get it. Have you tried the Ohio Department of Education website?
http://www.cheohome.org/ Click on link. In left hand column, click on Homeschool Laws. On the pop out tag, click on Assessment Forms. Choose which form you want on the next pop out tag. There are 2. One is for narrative assessment, the other is for standard achievement test assessment. It's a PDF so you should be able to make a copy from there or you can download it as a black to your computer to use every year. :love:
