I am trying to put together my progress reports from the past year and was wondering if anyone knows of any templates or examples out there to give me a better idea of what to include

The state of GA doesn't have guidelines for this. We just talked about this at one of my homeschool groups. Most people in the group said they simply write a paragraph or so about each subject saying what they covered. That's all. You don't have to turn them in to the state anyway (of course unless there is a legal problem one day)...silliness...anyway, you can use http://www.thehomeschoolmom.com/teacherslounge/educationese.php to help you word the report 'correctly' in educationese. If you're looking for premade forms you can print out try http://donnayoung.org/forms/index.htm (FREE) Some people prefer to use an online program throughout the year so they can just print out a report at the end of the year. Others also keep samples of the child's work from throughout the year. HTH
